Who is responsible for your data
Sumqayit Hospital is the operator of your personal data. Our contact details are at the end of this page.
We process your data in accordance with the Law of the Republic of Azerbaijan “On Personal Data” and other applicable legislation.
What data we collect
When you send the online appeal form:
- your full name;
- your phone number;
- your email address (optional);
- the doctor you chose and your preferred appointment date (optional);
- the type and text of your appeal;
- the language you wrote in and the time you sent the appeal.
Please include only the information needed to handle your appeal. Do not enter detailed medical records into the form — you can bring them to your appointment. For security, the website also keeps technical data (IP address, browser type, time of request) in server logs for a short period.
Why we use it
- to review your appeal and contact you;
- to arrange an appointment time and refer you to the right doctor;
- to look into complaints, suggestions and thanks, and to improve our services;
- to protect the website against abuse, spam and attacks.
We do not use your data for advertising and we do not sell it.
Legal basis for processing
By ticking the consent box before sending the form, you agree to your data being processed for the purposes described in this policy. Processing technical data is necessary for the website to operate securely.
Who can see your data
Appeals are seen only by hospital staff authorised to handle them. Your data may be stored on the servers of the hosting provider that runs the website; such providers may not use the data for their own purposes.
We disclose data to third parties only where the law requires it — for example, at the lawful request of a competent state authority.
Third-party services
Some parts of the website rely on external services, which are governed by their own privacy policies:
- Google Maps — the map on the Contact page;
- WhatsApp — the “Message on WhatsApp” buttons open the WhatsApp app;
- YouTube — videos embedded in posts;
- Facebook and X — the Share buttons.
These services may set cookies when you use them or when their content loads. The website itself does not use analytics or advertising cookies.
How long we keep it
We keep appeal data for as long as needed to handle the appeal, record its outcome and meet legal requirements. After that, the data is deleted or anonymised so that it can no longer identify you.
Security
We take technical and organisational measures to protect your data against unauthorised access, alteration and loss: data travels over an encrypted connection, and the administration panel is password-protected and open only to authorised staff. Even so, no transmission over the internet can be guaranteed to be completely secure.
Your rights
Under the law, you have the right to:
- find out what data we hold about you;
- ask us to correct inaccurate or outdated data;
- ask us to delete your data;
- withdraw your consent to processing at any time.
To exercise these rights, contact us using the details below. We respond within the time limits set by law.
Children
An appeal on behalf of a person under 18 should be sent by a parent or legal guardian.
Changes to this policy
We may update this policy from time to time. The new version takes effect as soon as it is published on this page; the date of the last update is shown at the top.
